Health Risk Assessment
Our primary goal is to help you and your family stay as healthy as possible. One way we do this is through the Health Risk Assessment (HRA).
The HRA is a short survey where we ask you questions about your health and lifestyle, and then we use your answers to develop a personalized care plan.
As a member, you can complete the HRA online by logging in to Healthy Connections and selecting the HRA tool.
