SafeRide Health (SRH) is the new provider for all NEMT rides to doctor appointments and pharmacy visits.
Depending on your needs, rides may include wheelchair-lift-equipped vehicles, stretcher vans, minivans, or ambulatory vans. Please let SRH know what type of ride you need when scheduling.
SafeRide Health (SRH) es el nuevo proveedor de todos los servicios de transporte médico que no son de emergencia (NEMT, por sus siglas en inglés) hacia consultas médicas y farmacias.

Call to action: Texas Children’s Health Plan (TCHP) is proud to present our Healthy Rewards Program/Value Added Services (VAS) program effective September 1, 2024 through August 31, 2025. These rewards and services are available in each line of business CHIP, STAR, and STAR Kids and Service Delivery Areas; Harris, Jefferson and North East (limitations may apply).
Member favorites, such as the Prenatal and Postpartum Visit Rewards, Portable Crib/Playpen, Diabetes Screening Rewards and Well-Child Visit Rewards are returning, along with a new benefit for our members with Asthma and ADHD. Please review the chart below for a full list of all rewards including updates to existing and new rewards.
As a reminder, if you are treating a CHIP-Perinate covered member with a diagnosis of hypertension during their pregnancy, you can request to have a blood pressure monitor mailed to their home. At this time, this is the only reward that can be requested by a provider.
Extra benefits are available for the following categories:
Receive a $50 reward for completing an HbA1C test for members 10 and up every 6 months Receive $25 reward card for completing diabetic eye exam once per year
Well-Child Checkup Reward (0-30mo)
UPDATE – include a longer age range and additional visits
Receive a $100 reward card for completing 6 well-child checkups by the age of 30 months.
Receive an additional $50 reward card when completing 2 additional well-child checkups by the age of 30 months
Young Adult Wellness Visit Reward (16-21yrs)
UPDATE - lower age limit from 18-21 to 16-21 years old - receive a $25 reward card for completing a yearly wellness visit.
Cervical Cancer Reward
Receive a $25 reward card for completing a cervical cancer screening during an annual well-woman exam. STAR only, age 21 and up.
Mental Health Follow-Up Visit Reward
Receive a $25 reward card for completing a mental health follow-up visit within 7 days after discharge from a mental health hospital or facility
Health Risk Assessment Completion Reward
UPDATE- STAR Kids Removed. Receive a $10 reward card for completing the Health Risk Assessment within 90 days of becoming a NEW member (CHIP and STAR only)
Screening and Assessment Instrument Completion Reward
NEW – receive a $50 reward card for completing the SAI screening within 90 days of becoming a NEW member (STAR Kids members only)
Asthma Education Reward
NEW – Members with a high risk Asthma diagnosis who enroll in and complete a 90-day case management program are eligible to receive a $25 reward card
ADHD Management Reward
NEW – Members age 6-12 who are newly diagnoses with ADHD, are prescribed ADHD medication and complete a 30 day follow—up visit after the initial diagnosis are eligible to receive a $25 reward card

How this impacts providers: Providers should encourage members to take advantage of the Healthy Rewards Program benefits/VAS that are applicable and helpful for them.
TCHP is continuing our partnership with Healthmine, branded Healthy Connections, for our members, for our VAS reward card fulfillment. Healthmine provides an online, interactive portal where members can actively participate in tracking their health actions and fulfill VAS gift cards. Please note, only the Prenatal, Postpartum, Flu Prevention, Cervical Cancer, Diabetes Screening Reward, Well-Child Checkup, HRA completion, SAI completion, ADHD Management, and Young Adult Wellness Visit rewards are fulfilled by Healthmine.
Members can request their reward and create their Healthmine (Healthy Connections) account a few ways:
For members age 18 and up, call Healthmine at 866-475-1619 (TTY711). Healthmine customer service agents can help members create their account and intake member VAS requests
For membersunder the age of 18, they must create their account online by either:
Logging into their MyChart account (under “Menu”, head to “Resources” and click on “Healthy Connections” to sign up)
Healthmine account creation and reward redemption user guides can be found on our Healthy Rewards website or by clicking here (available in both English and Spanish)
Please encourage members to create their Healthy Connections account (if not already done so) as this will be the only way members can request and redeem these rewards
