Eyewear Support

Receive an allowance towards upgrades of $110 for framed glasses or $90 for contact lenses and contact fittings.

Eligible members: CHIP, STAR, and STAR Kids members up to 18 years old.

Name and description of the benefit changed for clarity purposes - In addition to the covered free pair of glasses each year, receive $110 to upgrade or replace your glasses or $90 towards contacts and contact fittings.

Well-Child Checkups Reward

•  Receive a $50 reward card when your baby completes 3 well-child checkups by the age of 15 months.
• Receive an additional $100 reward card when your baby completes 6 well-child checkups by the age of 15 months.

Rewards can be requested up to 30 days after the end of the eligible year. Eligible members: CHIP, STAR, and STAR Kids members up to 15 months old who complete at least 3 well-child checkups.

 Updated number of visits and extended timeframe to qualify for the reward –

• Members now receive a $100 reward card when they complete 6 well-child checkups by the age of 15 months. Previous reward was $50 after 3 well-child checkups.
• Members now receive an additional $50 reward card when they complete 2 more well-child checkups by the age of 30 months. Previous reward was $100 after 6 well-child checkups by 15 months.

Eligible members: CHIP, STAR, and STAR Kids members up to 30 months old who complete at least 6 well-child checkups.

Young Adult Wellness Visit Reward

 Receive a $25 reward card when you complete a yearly wellness visit between the ages of 18 and 21.

Rewards can be requested up to 30 days after the end of the eligible year.

Eligible members: CHIP, STAR, and STAR Kids members who complete a yearly wellness visit between the ages of 18 and 21.

 Age range changed –

Members can now get a $25 reward card when they complete a yearly wellness visit between the ages of 16 and 21. Previous age range was 18-21.

Eligible members: CHIP, STAR, and STAR Kids members who complete a yearly wellness visit between the ages of 16 and 21.

Health Risk Assessment Completion Reward

 Receive a $10 reward card when you fill out the Health Risk Assessment form within 90 days of becoming a member.

Reward can be claimed up to 30 days after the end of the eligible year.

Eligible members: CHIP, STAR, and STAR Kids members who enrolled within the last 90 days.

Benefit no longer available for STAR Kids members. The Screening and Assessment Instrument Completion Reward below will now be available for STAR kids.

Eligible members: CHIP and STAR members who enrolled within the last 90 days.

Screening and Assessment Instrument (SAI) Completion Reward

NEW BENEFIT!

 Receive a $50 reward card when you enroll in the Case Management Program and complete the Screening and Assessment Instrument (SAI) within 90 days of becoming a member.

Reward can be requested up to 90 days after the end of the eligible year.

Eligible members: New STAR Kids members who enroll in the Case Management Program and complete the Screening and Assessment Instrument (SAI) within 90 days of enrollment.

Asthma Education Reward

NEW BENEFIT!

 Receive a $25 reward If you are a member with a new high-risk asthma diagnosis who has enrolled in and completed the 90-day asthma Case Management program.

Reward can be redeemed up to 90 days after the end of the eligible year. Eligible members: CHIP, STAR, and STAR Kids members with a high-risk asthma diagnosis who enroll in and complete the 90-day asthma Case Management program.

ADHD Followup Visit Reward

NEW BENEFIT!

 Receive a $25 reward card if you are a member who has been newly diagnosed with ADHD, is prescribed ADHD medication and completes a 30-day follow up visit after initial diagnosis.

Reward can be requested up to 90 days after the end of the eligible year.

Eligible members: STAR and STAR Kids members ages 6 to 12 who have been newly diagnosed with ADHD, are prescribed ADHD medication and complete a 30-day follow up visit after initial diagnosis.
